Provider Update: April 1, 2016
Reminder regarding the termination of the Service Plan Authorization priority fax line (405-230-8019).
Per the MSU Update sent March 18, 2016, the Service Plan Authorization priority fax number will become inactive effective 04/04/2016.
Service Plan Authorization requests faxed to this number after April 3rd will not be received.
Priority requests can only be received through SmarterMail; there is currently no fax option.
Please advise your staff that ALL priority requests sent on or after 04/04/2016 must be submitted to the following email address via Smarter Mail:
