Subject:Blank Signature Page Templates

For Use From 8-18-18 through 8-26-18 Only

Attached please find two blank signature page templates that will need to be used separately for both ADvantage Members and State Plan Personal Care (SPPC) Members between the dates of 8-18-18 through 8-26-18. These signature pages will be utilized for any IDT/plan development visits that fall between these dates.  Beginning 8-27-18 you will need to upload the signed sheets into the Harmony system, along with entering all information that occurred during this timeframe for those Members that needed an IDT or plan development during these dates.

For Use Beginning 8-27-18 and Forward

Once the go-live Harmony date of 8-27-18 has occurred, these templates will no longer be utilized.  Instead, you will utilize the sign in sheets located in the Harmony system.  These documents in Harmony are a cleaner version that will auto-populate the Member information at the top of the document.
