Agency Conversions Suspended

Implementation of Conversion Suspension

Due to the implementation of the new EVV system, beginning immediately MSU-AA Contracts will be suspending all agency conversion requests until November 1, 2014.

Agency conversions include, but are not limited to the following:

Branch Changes (adding, removing or closing branches)
Change in Provider’s Federal Employer’s Identification Number (FEIN)
Agency Closures
Agency Purchases
County Transfers

Should any of the agency conversions listed above occur during this suspended time period, please prepare your written notification and submit to the MSU-AA, via certified mail, as usual. We will begin implementing those requests received during the suspension period on November 1, 2014, in the order they were received. At that time, Contracts will contact you regarding disposition of your request.
