Black Heritage Award nominations due Dec. 14, 2012

OKLAHOMA CITY -- The Black Heritage Committee, sponsored by the Oklahoma Department of Human Services (OKDHS) Office for Civil Rights, is accepting nominations for several awards that will be presented at the 28th Annual Black Heritage Celebration on Feb. 15, 2013. The event will be held at the Metro Technology Center, 1900 Springlake Drive in Oklahoma City, beginning at 11 a.m.

The celebration is a partnership between the OKDHS Office of Civil Rights, the Oklahoma Department of Corrections and the Oklahoma Historical Society. The theme of this year’s celebration is “Bringing A Generation Into Focus.”

The following awards will be presented at the event:
 

• The Meritorious Service Award will be presented to the OKDHS employee who has exhibited outstanding contributions to the agency and the community.
• The Trailblazer Award will be given to the person the Black Heritage Committee defines as “going where no man or woman has gone before” by making groundbreaking achievements in any fields or endeavors. 
• The Lifetime Achievement Award will be presented to the person whose contributions to Oklahoma and/or the nation have been remarkable.

All nominations should include a description of why the nominee should be selected. The completed Black Heritage Award Nomination Form (.pdf, 1p, 216 KB) should be emailed to peggy.carter@doc.state.ok.us. Nominations must be received no later than noon on Dec. 14, 2012.

Nominations for the Meritorious Service Award are open only to OKDHS employees, but nominations for the Trailblazer Award and the Lifetime Achievement Award are open to anyone.
