OKLAHOMA CAPITOL -- The ADvantage Program has recently moved from the Long-Term Care Authority of Tulsa to the Oklahoma Department of Human Services-Aging Services Division. With this recent change, which will be completed Oct. 31, 2008, contact numbers for the ADvantage Program have changed.
Please note the following changes effective immediately:
ADvantage main telephone number has changed from (918) 583-3336 to (918) 933-4900.
In addition, the following ADvantage fax numbers have changed effective immediately:
- Receptionist/Referral Fax has changed from (918) 583-0426 to (918) 933-4980;
- Claims Center has changed from (918) 583-0742 to (918) 933-4981;
- Mail room has changed from (918) 849-5201 to (918) 933-4982; and
- ADvantage Operations has changed from (918) 879-5316 to (918) 933-4983
The ADvantage Program of the Home-and Community-Based Services Unit provides Medicaid services to help people stay at home instead of going to a nursing home. The program assists frail elders and adults who have physical disabilities but not children or persons with mental retardation.
