OKLAHOMA CAPITOL --- Because of recent concerns about end of life issues, the Oklahoma Department of Human Services Aging Services Division would like to remind all Oklahomans that free Oklahoma Advance Directive forms are available on the OKDHS Web site or by mail.
Simply go to http://www.OKDHS.org and click on the “Advance Directive for Health Care” link under OKDHS Resources. The form is also available in Spanish and Vietnamese.
For those with limited or no access to the Internet, a copy of the form can be mailed to you by calling the OKDHS Records Center at 1-877-283-4113. For free legal assistance in completing the form, contact the OKDHS Aging Services Division at (405) 522-3069.
