317:35-9-27. Application procedures for public ICF/MR

When an individual is admitted to a public ICF/MR, an application for payment of long-term care in the facility is made at the time of admission. A designated worker from the county office in the county where the facility is located assists in this part of the admission process. The superintendent of the facility may sign the application on behalf of the individual if the responsible parent or guardian is not available. A case record is set up, in the county where the facility is located, for each applicant of the public ICF/MR. •1 If the individual leaves the facility, the county case is transferred, if necessary, to the county of residence.

Case information on each resident of each of the facilities is entered into the computer by the designated worker as soon as the needed statistical information is available following admission.
