317:30-5-235. Documentation

     All services must be documented in the member's medical record.  All prenatal and postpartum lactation sessions must, at a minimum, include the following:

• (1) date of service;

• (2) start and stop time for each session;

• (3) documentation of services provided;

• (4) practitioner's signature; and

• (5) recommendation and plan of care.
