What form(s) do I need?

For complaints about discrimination, there are two forms. The first, Form 14CR019E - Employee Discrimination Complaint Form is used by DHS employees to file a complaint of discrimination. The second, Form 14CR001E - Client or Vendor Discrimination Complaint is used by clients or vendors doing business with DHS and who are filing a complaint of discrimination.

For filing a request for Reasonable Accommodation, use Form 14CR003E - Request for Reasonable Accommodation. This form can be use by DHS employees, clients or vendors filing a request for Reasonable Accommodation.

How do I file a complaint?

Complete all information on the designated form. Return the form by mail to the DHS Office for Civil Rights, PO Box 25352, Oklahoma City, OK 73125-0352 or by fax to the DHS Office for Civil Rights at (405) 522-3197.
