Forms must filled out electronically.

REQUESTS MADE BY VENDORS WILL BE REJECTED. ONLY REQUESTS MADE BY ENTITIES WILL BE ACCEPTED.

Name Description
Change Request The Change Request Form must be completed to request any changes to existing radio entries or talkgroup entries in the OKWIN system. It can also be used as a Letter of Authorization (giving permission to an agency/entity to use another's agency/entity's talkgroup in their radio. Must be filled out by the authorized representative of the agency/entity GIVING permission.)
ID Request

The ID Request Form must be submitted for any new radios to be added to the OKWIN system.

NOTE: This form must be approved by the Sponsor prior to the purchase of any radios.

New Account Form If you do not have an account with the DPS Radio Operations Center and would like one please fill this form out.
Outage Notification Form This form will send a report to an OKWIN administrator or assigned on-call personnel. Please fill out the form completely so that the appropriate corrective actions can be accomplished expediently. Thank you for keeping us informed.
