Forms
Forms must filled out electronically. They WILL NOT be accepted if they were physically signed and submitted. They must be signed electronically and then emailed to okwin@dps.ok.gov.
REQUESTS MADE BY VENDORS WILL BE REJECTED. ONLY REQUESTS MADE BY ENTITIES WILL BE ACCEPTED.

Change Request* | The Change Request Form must be completed to request any changes to existing radio entries or talkgroup entries in the OKWIN system. It can also be used as a Letter of Authorization (giving permission to an agency/entity to use another's agency/entity's talkgroup in their radio. Must be filled out by the authorized representative of the agency/entity GIVING permission.) |
ID Request* | The ID Request Form must be submitted for any new radios to be added to the OKWIN system. NOTE: This form must be approved by the Sponsor prior to the purchase of any radios. |

*The files will be in .zip format. You should be able to unzip and save the PDF document.
PSA: We are transitioning to a more user friendly system. Please Use the New Request form or the Validation Form. We will continue to use the Old Change Request and ID Requests, however, soon they will no longer be available.
NOTE: If your entity is currently not on the OKWIN network, please email okwin@dps.ok.gov with a request to be added to the system as a new subscriber, detailing reasons and radio models.
