First, you must have personal knowledge of the condition, such as that acquired by a family member, doctor, judge, or law enforcement officer. You will then need to complete a Request For Driver Review form. This will permit us to conduct an investigation concerning the driver's driving and health issues. 595:10-5-1. Purpose
To operate a motor vehicle, a driver shall possess certain physical and mental abilities. The first and primary purpose and intent of the Department of Public Safety is to license new applicants and to renew driver licenses of persons with these abilities but, when necessary, to restrict, deny, cancel, or disqualify the driving privilege of those applicants or licensees who present an unwarranted risk or do not meet the minimum standards.
