An appeal may be submitted within thirty (30) days to the Ignition Interlock Program Administrator for the following actions:

(1) denial for an IDAP completion certificate; or

(2) a program failure; or

(3) a denial of eligibility for affordability accommodations; or

(4) a denial for medical exemption; or

(5) a denial for employer exception.
