Computer-Fillable Collision Report Form

The form of the report required by this section shall be prescribed by the Commissioner of the Department of Public Safety.

The four main pages of the collision report form are composed as one file. Supplemental pages are each in a separate file.

  1. The computer-fillable form must be downloaded to your computer to perform properly. The form, including a diagram, is to be filled out, and printed as usual. You will need the latest version of Adobe Acrobat Reader® which is available at no charge from Adobe's website.
  2. You will be able to use the "Save package as" command, under the "File" menu to save the first four pages as a group if you are using Adobe Reader 8 . If you are using Adobe Reader 7, each page will need to be saved individually.
  3. Remember this form is specially designed to be scanned by the Records Management Division at DPS and is the only officially authorized form. Self-generated forms are not acceptable. The better quality the form, the better quality scanning can be achieved.
