Critical Care Needs: Special needs (such as neuropsyhological evaluations for veterans who have all the medical evidence of head trauma) for veterans who cannot get the VA to conduct comprehensive evaluations, nor acknowledge their issues as service connected.

Quality of Life Upgrades for Resiedents: Power charis and other mobility equipment, updated recreation area, vehicles, local travel, life experience events, modern entertainment equipment, exterior beautification, hands-free technologies.

 

Oklahoma ODVA Veterans Homes Application Process

You can start your application for an Oklahoma State Veterans Home by completing the official ODVA “Request Information” form, which will connect you with a Patient Services Coordinator at your chosen facility.

Step-by-Step Process

1. Select Your Preferred Veterans Home
Oklahoma’s ODVA operates multiple state-run facilities, including Ardmore, Claremore, Clinton, Lawton, Norman, Sallisaw, and Sulphur. Choose the home(s) you are interested in when you submit the form.

2. Complete the “Request Information” Form
Go to the official ODVA Veterans Homes page and fill out the Request Information form online. You will need:

  • First and last name

  • Phone number of the responsible party

  • Email address

  • Selected Veterans Home(s)

  • Urgency of placement (“As Soon as Possible,” “Sometime in Future,” or “Other”)

  • How you heard about ODVA

  • HIPAA policy acceptance

  • Electronic signature

3. Contact a Patient Services Coordinator
After submitting the form, a Patient Services Coordinator from your selected home will contact you directly to discuss eligibility, available spots, and next steps.

4. Eligibility & Admission Requirements
While ODVA does not publish a full checklist in the form, the Oklahoma Veterans Homes site notes that facilities are federally approved by the U.S. Department of Veterans Affairs and licensed by the Oklahoma State Department of Health. Common requirements include:

  • Being a U.S. veteran (or eligible spouse/parent)

  • Medical need for long-term care (often with a physician’s recommendation)

  • Meeting facility-specific criteria.

5. Gather Required Documentation
Although the ODVA form does not list all documents, many state veterans homes require:

  • DD Form 214 (military discharge papers)

  • VA Form 1010EZ (health benefits application)

  • Current insurance card

  • Other relevant legal documents (e.g., power of attorney).

6. Placement & Admission
Once the coordinator confirms eligibility and space, you will be scheduled for an admission process, which may include a home visit, medical review, and paperwork completion.
