SW1025 Supplier Information

NOTE:  Prior to sending an email with the below required information, you must register first with Vendor Registration.

We are working to automate the on-line functionality for the supplier application.   In the interim, e-mail the following information to

  • Supplier Business Entity Name

  • Supplier Business Address

  • Supplier Contact Name

  • Supplier Contact E-mail

  • Supplier Contact Phone Number

  • Authorized Signer Name for Master Service Agreement

  • Authorized Signer E-mail

You will be contacted once Central Purchasing receives the e-mail with the required information.
