SW1025 Supplier Information

NOTE: Prior to filling out this form, you must register to be a supplier through the Oklahoma Supplier Portal.

We are working to automate the online functionality for the supplier application. In the interim, email the following information to

  • Supplier business entity name.

  • Supplier business address.

  • Supplier contact name.

  • Supplier contact email.

  • Supplier contact phone number.

  • Authorized signer name for Master Service Agreement.

  • Authorized signer email.

Central Purchasing will contact you once the email with the required information is received.
