SW1025 Supplier Information

NOTE: Prior to filling out this form, you must register to be a supplier through the Oklahoma Supplier Portal.

We are working to automate the online functionality for the supplier application. In the interim, email the following information to SW1025.Supplier@omes.ok.gov:

Supplier business entity name.
Supplier business address.
Supplier contact name.
Supplier contact email.
Supplier contact phone number.
Authorized signer name for Master Service Agreement.
Authorized signer email.

Central Purchasing will contact you once the email with the required information is received.
