Welcome to the ADvantage long-term care program! We are delighted that you chose this program, which allows you to receive home care services while you remain in your home with help from family and friends. Some of the services the ADvantage Program offers are: Case Management, Personal Care, Home-Delivered Meals, Specialized Medical Equipment and Supplies, Skilled Nursing, Adult Day Health, Assisted Living, and CD-PASS (Consumer Directed Personal Assistance Services and Supports).

The ADvantage Program is a good alternative to other long-term care options like nursing home care.  Since the ADvantage Program does not provide 24-hour care, you must be able to safely remain in your home.

When you were assessed for the ADvantage Program, you chose agencies to provide case management and home care services. We have contacted the agencies and instructed them to contact you as soon as possible. Agency staff will meet with you and your family or friends to coordinate a care plan that will meet your specific needs. For you to achieve the highest level of health and well-being, it is important for you to actively participate in planning your care and share information with your case manager and home care aide. Your family members or friends are also encouraged to participate in the care planning, should you choose.

If you have any questions regarding the program, call the ADvantage CareLine at 800-435-4711. The CareLine is open 8 a.m. to 5 p.m. from Monday through Friday. We want you to have the best experience possible with the ADvantage Program, so please let us know if we can improve anything to serve you better.   

Our goal is to assist you in being as independent as possible, for as long as possible.
