317:30-5-17. Authorized examinations - eligibility determinations

     When an examination is to be made for the purpose of determining original or continuing eligibility, it is necessary that DHS Form ABCDM-16 (new form number 08MA016E), Authorization for Examination and Billing, be secured from the county office of the Department of Human Services.  Report of such examination must be submitted on DHS Form ABCDM-80 (new form number 08MA080E).  DHS Form ABCDM-16 (new form number 08MA016E) must be attached to the claim.  If a UA is indicated, payment will be made separately at the current allowable rate.
