FACS Case Notes
Case notes can illuminate even the most difficult cases. You should read at least back to the last certification (for CRs) – which would include the MCR for benefit reporters. For applications with a large break in services, it is still a good idea to read the last few notes to see why a case closed last time.
Good case notes will show you what processes other workers have done. It also shows the logic other workers have used to certify the case. It is important that you also create detailed case notes for every action that you do. Even if you are responsible for every action, it is important that you continue to keep good case notes in case another worker becomes responsible for this case. It is to your benefit as well, as 6 to 12 months is a long time between renewals.
