CR Notice Form
In addition to including instructions on timeframes and methods of application, the notice also includes:
- A message to clients informing them that they must provide information to complete the CR within a 30-day timeframe.
- Clients are not eligible for expedited services
- Just like an initial application, you must complete an interview as soon as possible to ensure timeliness of benefits.
- Information on the OKDHSLive! website and the Support Center phone number: 405-487-5483.
- How to obtain a paper renewal form.
- A message to clients informing them that they will be contacted for an interview.
- Notification that household income must be provided.
- Notification that proof of certain expenses is required before a deduction is given.
