Please include the CLIA number on your request as well as a contact person and their phone number. Please be aware a new certificate will not be issued automatically. If a new certificate is needed, please see below.

for a replacement certificate. Please include the CLIA number as well as the name of a contact person and their phone number.

 Your new CLIA certificate will arrive approximately 2-3 weeks prior to your expiration date.

Please submit your request to change the director on your CLIA certificate on the CMS-116. All 5 pages must be filled out completely and signed. The form can be emailed to medicalfacilities@health.ok.gov or faxed to (405) 900-7559.  If you have any questions, please contact us via email or at (405) 426-8470.
