Before applying for the exam an applicant must first complete an approved training course and submit an application to the Oklahoma State Department of Health (OSDH). Once the OSDH approves the application, the applicant will receive a letter with instructions to sign-up for the certification exam.

You must contact your preferred testing center from the list to schedule a testing appointment. The testing center should advise you of their acceptable methods of payment at the time of scheduling.
The fee for the examination is $200.00. Payment must be made directly to the selected testing center to avoid any delays in the examination process. Please do NOT send payment to the Oklahoma State Department of Health. Candidates who do not pass the test must wait seven (7) days before retesting; the fee for retesting, if required, is $75.00.
On your scheduled test day, you must present two documents to the scheduled testing center:
- The original exam certification letter from the Oklahoma State Department of Health documenting eligibility to test.
- An unexpired driver’s license or other photo identification issued by a U.S. government entity.
NOTE: Testing will be refused if both documents are not presented.
