How to Apply for a Tattoo and/or Body Piercing Establishment License
An establishment performing tattoo or body piercing art must obtain a license before they open. A license is required for each art performed in the establishment.
Application
To apply for licensure, please complete the Body Piercing / Tattoo Establishment License application and mail in with the appropriate fee(s) and documents.
- Affidavit of Lawful Presence / Legal Residency
- Consent Form
- Written Aftercare Instructions
- Proof or Articles of Incorporation (only if the owner is incorporated, such as LLC, Inc, etc.)
If applying for both arts, separate applications must be submitted; however, they may be mailed together with one check for the overall total.
Fees
Fees must be received with the application before an initial license inspection will be scheduled.

Initial License Inspection
Before opening the establishment, an inspection must be conducted
- Include on your application a 1-to-2 week timeframe when you know the shop will be ready to open.
- The inspector will contact you to schedule an inspection once a complete application and fee have been received by the Department.
- Do NOT schedule an inspection before you have everything complete.
- Use page two of the initial license application as a tool to help ensure shop requirements are met.

Please note, you will not receive a license approval until the shop meets all requirements outlined in Oklahoma Administrative Code (OAC) 310:233 Body Piercing and Tattooing.
