Community Health Assessment

Cleveland County began working on the update for our third iteration of the Community Health Improvement Plan in the Fall of 2018 using the Mobilizing for Action Through Planning and Partnerships (MAPP) model. The goal of this process was to formally assess the health status of the county in a comprehensive way, and solidify a formal approach for community health improvement efforts.

2019 Cleveland County Health Assessment

The community health assessment is a collaborative process of collecting and analyzing data and information for use in educating and mobilizing communities, developing priorities, garnering resources, and planning actions to improve the population's health.

Substance Abuse 

Objectives are decreasing the rate of deaths in alcohol-related car crashes and the rate of unintentional poisoning deaths

involving prescription drugs.

  • Lead Organizations:  OU Southwest Prevention and Norman Police Department
  • Meets monthly on the 2nd Thursday from 1:00-3:00.

Mental Health

Objectives are increasing access to mental health care and healthy lifestyle.

  • Lead Organizations:  ODMHSAS-Griffin Memorial Hospital & Transition House, Inc.
  • Meets monthly on the 2nd Friday from 10:30 – 1:00

Healthy Living

Objectives are to reduce the obesity rate, increase fruits and vegetable consumption and increase physical activity among Cleveland

County adults.

  • Lead Organizations: OU College of Health and Exercise Sciences & Cleveland County Health Department
  • Meets the 3rd Thursday every other month from 2:00-4:00

Child Health

Objective is to improve the health of children in Cleveland County.

Access to Care
In 2013, the Cleveland County Health Department began collaborating with community partners to address existing barriers to accessing health care services in Cleveland County. The process began with an assessment of health care access which was conducted during April 2013 in communities throughout the county. A workgroup was formed and are currently prioritizing those issues to be addressed.

The Cleveland County Access to Healthcare assessment results are now available.
