Oklahoma Birth And Death Certificates

Oklahoma vital records maintains the records for births and deaths that occurred in the state of Oklahoma.

Vital Record Lobbies are Closed

Out of an abundance of caution due to increasing COVID-19 cases around the state, the lobbies have been closed across the state to reduce the risk of exposure.

Please consider placing your order online or by phone. These orders are typically processed the next business day.

You may also submit your application by Mail. Complete mail orders are typically processed within 4 weeks. Be sure to review before sending by mail to assure your order is complete. Missing items or information will delay our response.


Online and phone orders

Birth and Death Certificates issued to eligible birth and death applicants. Cost is $20.00 for first copy and $15 for each individual copy for Oklahoma residents. Online rates will vary for non state residents. Orders may be expedited for an additional fee.

Mobile App (MyOklahoma)

Orders for birth certificates are limited to a named parent or the subject of the record. Cost is $20.38 per copy ordered.

Amendments Only

Cost is $13.95 processing fee,plus $40 for a birth record amendment and issuance of one certified copy. Orders may be expedited for an additional fee.

Mail (complete orders issued within 4 weeks)

Required: 1) Application birth death, 2) Photo ID, and 3) Fees birth death checks or money orders should be payable to OSDH.

4) Birth and death records are confidential. If you are not a named parent or the subject of the record, you will need to include additional documentation before a record can be released. birth death.
