The child support computation form is the legal document used to calculate the child support obligation for a noncustodial parent (obligor) as required pursuant to Section 120 of Title 43 of the Oklahoma Statutes. The form must be signed by the judge and attached as an exhibit to orders which establish or modify a child support obligation. The Insurance Premium Worksheet is used to compute the monthly health insurance premium for the children in the case. Health insurance is reasonable in cost when the share of the premium for the parent providing the coverage does not exceed 5% of that parent’s gross monthly income. This worksheet is embedded in the Excel calculator and available below as a fillable form.

  • When opening the .pdf form from a web-browser such as Firefox, Microsoft Edge, or Chrome:
    • download the form - right click on the link and select save link as and save it to your computer;
    • open the file  - right click on the file and choose open with Adobe Reader.
  • If you are using Chrome, updating your settings will help you view these forms.
    • Select Settings in Chrome > Select Extensions > Find your Adobe Reader extension here and turn the switch from “off” to “on.”
    • After this, selecting one of these forms should open the form in Chrome and give you the option to view it in Adobe without downloading first
